The times it has happened to me were because after the preview I failed to Submit.

I know that some posts have been lost during an upgrade of the system - but that rarely happens.

I'm thinking it is because the submit button does not get clicked or maybe I hit the post reply button or just because. All my technology is Mac...both iMac and iPad. I know all my posts are extremely important and some of the more significant ones never make to the site; just so I haven't been put put on the watch list.

From time to time, I have had it happen too. Sometimes it is because someone has submitted a new post while I was posting. You need to stay there and be sure that your post was submitted before you click 'submit' and go. Create all of your posts using the 'Full Editor & Preview'. There is an automatic save function there. If you do that and your post doesn't show, then just hit the back button until you are on the page where you created the post or go to the 'Load quick draft' and then try to post it again.

Yeah I can see issues there...I have had posts not go through due to...lets see if I remember, invalid form submitted, from waiting too long (hours), and another user has replied and you might want to change your post. (which I totally disagree with the very concept) . Both the errors have caused my not go through, and depending on the browser button clicking, they could get lost.

I've seen similar issues in the PM system and some odd things get barfed up into the subject line from time to time. And having to resend something because it didn't go.

But then, I am 100% non-political. The story may be originally written with an agenda in mind but knowing how sensitive some people are about almost everything my internal censorship button deletes everthing questionable so when it is in doubt !@#$%^&*(*())__+_+.
